Step 1 Get the Rounded Retangle-Tool and set the radius to 5 px and draw a shape like mine;
Right click, press Make Selection and fill it with medium gray on a new layer;

Step 2 Cut off pieces like me, then go to Filter>Noise>Add Noise and choose a small noise-amount;

Step 3 Make sure it looks like this when you delete small pieces of the retangle;

Step 4 Add these layer-styles to the floppie;

Step 5 Add a black square behind the floppie;

Step 6 Now we are gonna dd the holes;
Use the Rounded Retangle-Tool to create 2 shapes;
The upper must have a radius of 10 px and the lower 20 px;
The shapes MUST BE BIGGER then the floppie;
Press CTRL+G to make them mask the floppie;

Step 7 Lower the opacity to 7-15 and add this layer-styles to it;

Step 8 Add the basic shape of an arrow with the same layer-styles as the (formal)white area's;

Step 9 With a radius of 20 px, create a white rounded retangle with a retangle CUT OUT OF IT;

Step 10 Press CTRL+G again and this time add a gradient, or change the hue by pressing CTRL+U;
Note:YOU CAN DO ANYTHING YOU LIKE, FROM GRADIENT OVERLAY'S TO JUST PLAIN COLORS.

Step 11 Now it's time to add a sticker. I've chosen for a 10 px radius rounded retangle with my logo on it ;)
